Thyroid problems are common, easily tested and highly treatable — but frequently either missed or over-treated.

Symptoms that point to the thyroid

An underactive thyroid slows the body down; an overactive one speeds it up. Both are easily confused with stress, anaemia or simply being busy.

  • Underactive: fatigue, weight gain, cold intolerance, dry skin, hair fall, constipation, heavy periods, low mood
  • Overactive: weight loss despite eating well, palpitations, tremor, heat intolerance, anxiety, loose motions, disturbed sleep
  • Either: neck swelling, irregular periods, difficulty conceiving

Testing and treatment

Diagnosis rests on TSH with free T4 and, where relevant, free T3 and thyroid antibodies. A single borderline TSH is not treated in isolation — it is repeated and interpreted alongside symptoms.

Hypothyroidism is treated with thyroxine, adjusted by repeat testing until the dose is right. Hyperthyroidism requires evaluation of the cause before treatment, and may need medication or referral for definitive therapy.

Thyroid in pregnancy and PCOS

Thyroid targets in pregnancy are tighter than usual and doses often need to be increased early. Thyroid dysfunction is also common alongside PCOS and can worsen weight and cycle problems, so both are checked together.

Frequently asked questions

Do I have to take thyroid medicine for life?
Often yes for hypothyroidism, because the gland's underlying capacity does not usually recover. Some cases — after certain infections, in pregnancy, or drug-related — are temporary and the medicine can be withdrawn under supervision with repeat testing.
How should thyroxine be taken?
On an empty stomach with water, at least 30–60 minutes before food, and separated from calcium and iron supplements by several hours. Taking it incorrectly is one of the most common reasons a dose appears not to work.
How soon should the test be repeated after a dose change?
Usually six to eight weeks. Testing earlier gives a misleading result.
